Treatments & Diagnosis for Reconstructive Urology
Reconstructive urology deals with repairing structural problems in the urinary tract and genital system, cases that go beyond routine treatment and call for advanced surgical technique. It's a discipline that borrows as much from plastic and reconstructive surgery as it does from general urology, since the goal is often to rebuild function and form together, not just remove a problem.
These are typically complex, open surgical procedures, often needed after trauma, congenital conditions, prior failed surgery, or long-standing disease, and they require both surgical precision and a clear understanding of how the urinary system needs to function afterward.
Key Areas of Reconstructive Urology
- Ureteric Reconstruction: When part of the ureter is damaged or diseased, we repair it using techniques such as a psoas hitch, Boari flap, or transureteroureterostomy, choosing the approach based on where the defect is and how much of the ureter is affected. In more extensive cases, the ureter may need to be replaced entirely using a segment of ileum, or repaired through ureteroureterostomy.
- Bladder Augmentation and Substitution: These procedures enlarge or replace bladder tissue and are most often needed for neurological conditions affecting bladder function, bladder cancer, or tuberculosis-related damage. Depending on the case, this may involve a continent urinary diversion (allowing the patient to self-catheterize) or an intestinal conduit diversion. - Urethral Reconstruction: For urethral strictures, whether from injury or scarring, or for hypospadias, we use anastomotic or substitution urethroplasty depending on the length and location of the affected segment. Buccal mucosal graft urethroplasty, using tissue from inside the cheek, is performed as either a single-stage or staged procedure depending on complexity.
- Additional Reconstructive Procedures: We also repair vesicovaginal and urethrovaginal fistulas, along with scrotal and penile reconstruction, for a range of urological and genital conditions that require a tailored surgical approach.
